Installed and reinstalled plugin multiple times.
URL field is missing
The 3 widgets are not loading
QR code is generated, but is unusable since it does not have a URL to which to link
Mobile version of site no longer usable

Check browser console for JavaScript errors and server PHP error log for errors in code, and post it here as screenshots or uploaded ZIP with errors. Also, please, provide screenshots of the widgets since I don’t understand how it is working if widgets are not there.

Everything you described point to an error, so it has to be there. Can you describe exactly what you did, and how you added QR code at all?
